- Combine milk and sugar in a medium saucepan over medium heat.
- Stir constantly until the sugar fully dissolves (about 2–3 minutes).
- Bring to a gentle simmer — do not boil. Reduce heat to low.
- Simmer uncovered, stirring occasionally, for 30–40 minutes. The mixture should reduce by about half and thicken slightly. It will thicken more as it cools.
- Remove from heat. Stir in butter and vanilla (if using) until smooth.
- Let cool, then store in a glass jar or airtight container in the fridge for up to 1 week.
